As a courtesy to you, a claim will be filed to your insurance company. Many insurance companies list specific instructions on the card regarding pre-certification or prior authorization for services. It is your responsibility to ensure that an authorization and/or pre-certification is obtained prior to services rendered and payment is received based on your contracted benefits with your insurance company. If authorization is denied or not obtained and payment is not received by your insurance company, you may be liable for any unpaid charges.
You will be required to pay any deductibles or co-insurance dictated by your insurance plan at time of service.
Our staff will call your insurance company to get the best estimate of what you will owe for the procedure. Please remember this is just an estimate. The final patient amount owed will be determined after your insurance company processes your claim.
We accept all major insurance plans. Participating insurances and their names change frequently. If a particular insurance is not listed, please call one of our local imaging centers to verify participation.
If you are not covered by insurance, Florida Diagnostic Imaging provides affordable self pay pricing. Payment will be due at time of service and if paying in full, an additional prompt pay discount is available.

We often think that health insurance will pay for all our medical needs, no matter why we need care. But, that’s not always the case when you file a lawsuit for personal injury, accident or a denied worker’s compensation claim. At Florida Diagnostic Imaging, we work with a number of funding companies that work with you to provide medical bill payments for personal injury, accident or denied worker’s compensation claims which enables you to get access to the healthcare you need, when you need it.
